Overview
The Speech-Language Pathologist will follow appropriate practices to perform evaluations, timely re-evaluations and OASIS assessments, determine SLP diagnoses, develop a plan of care, and perform SLP procedures to aid in restoring/maintaining speech, language, motor, and swallowing abilities. This position must also be able to follow and adhere to the practices described in the Department Policy and Procedure Manual. The Therapist is required to be flexible with daily scheduling and may be required to work various patient care settings . May need to assist in coordination of Home Health Aides, participation in interdisciplinary case management, completing quality chart reviews, participating in meetings as required by facility/department, and engagement in other initiatives to improve quality outcome.
