The ACTT Licensed Practical Nurse will assist and support individuals who are served in the development of skills, behaviors, responsibilities, and supports needed to live, learn, work and be integrated into the community with the greatest possible degree of self-determination and independence. This person will provide nursing services including administration and monitoring of medication, nursing assessments and interventions.
• Deliver nursing services, case management services, and supportive counseling, under the supervision of a registered nurse, to the individuals served by the Assertive Community Treatment Team.
• Provide medication monitoring, medication administration and medication education.
• Review laboratory values and report to team psychiatrist.
• Evaluate physical health and make appropriate referrals to community physicians.
• Order and monitor medical supplies.
• Evaluate potential bloodborne pathogen exposure incidents.
• Link individuals with the Assertive Community Treatment Team psychiatrist.
• Facilitate training of independent living skills, establish access to community services and agencies, assist individuals in establishing support networks in the community and assess individual needs on a daily basis.
• Coordinate services with the program, staff, inpatient facilities, and detoxification facilities and establish linkage to residential programs, self-help programs and community resources.
• Review treatment progress and monitor and provide continuous consultation with other Assertive Community Treatment Team staff and community agencies to ensure the consistent therapeutic management of the individual’s needs.
Associates: Nursing, High School Diploma (Required)
Drivers License (Valid) - USA, Licensed Practical Nurse (LPN) - State Board of Nursing
Experience working with adults with a mental health diagnosis and/or substance use disorder | 1 Year | Not Required
