Job Description Summary
The House Calls Advanced Practice Provider (APP) is responsible for complex clinical management of patients in a home setting. An advocate for the patient, the APP will provide total care for the patient in collaboration with the House Calls Medical Director and leadership team. The APP will ensure that timely, patient-centered, appropriate evidence-based medicine is provided.

Essential Duties and Responsibilities
- Provides clinical medical management to meet patient needs and prevent unnecessary Emergency Department utilization when appropriate. In home services include but are not limited to: IV hydration/antibiotic therapy, nebulizer treatments, wound care, and injections, and utilization of mobile diagnostics as indicated.
- Participates in the on-call rotation providing after hours telephonic support to patients (approximately 6-10 shifts/year)
- Ensures all pertinent and active medical conditions are documented in the electronic medical record in a timely manner, consistent with MPG policy, and in a compliant manner with CMS and other insurance carriers.
- Will address outstanding Quality Measures and HCCs as applicable to meet or exceed all quality goals. The APP will complete all documentation and billing in a timely manner, per protocol. Responsible for other duties as assigned.
- Exhibits strong clinical skills to improve patient care for an aging, adult population; as well as excellent interpersonal relationship skills in interpreting and communicating health care information to the patient and family.
- Accountable for identifying patients needing additional assistance and making referrals to home health, social services, behavioral health, skilled nursing and other available community resources, when needed and appropriate.
- Attends and participates in interdisciplinary meetings as requested to improve patient clinical outcomes.

- Education and/or Experience - Completion of master's degree in nursing.
Computer Skills - To perform this job successfully, an individual should have knowledge and experience with documentation in electronic health records and healthcare related communication platforms.
