Position Details
Location: Concord, CA (Contra Costa County)
Pay: Starting at $60/hour DOE
Schedule Options: Full-Time, Part-Time, and PRN available
Flexible Scheduling: Choose from Tuesday–Saturday or Sunday–Thursday schedules

Key Responsibilities
Conduct evaluations and create individualized treatment plans in collaboration with physicians
Supervise Physical Therapist Assistants in accordance with California State Practice Act guidelines
Collaborate with nursing and interdisciplinary teams to support patient progress and discharge planning
Recommend appropriate DME and support safe patient transitions home
Educate patients, families, and staff on safety, mobility, and therapeutic programs
