Job Description Summary:
Responsible for medication management, dispensing, and patient/family education. Supports patients seen in ambulatory clinics, hospital discharges, hospice/palliative care, and supports ongoing medication maintenance in the home. Rotating pharmacist on-call responsibility required.

Education Requirement:
Doctor of Pharmacy, required. Candidates who have been licensed and practicing as a pharmacist since before 2004 may qualify with a bachelor’s degree.
Licensure Requirement:
Licensure, or licensure-eligibility, to practice pharmacy in the state of Ohio, required.
Skills:
Experience:
Homecare, Home Infusion or Hospital experience or internship, preferred.
Physical Requirements:
OCCASIONALLY: Bend/twist, Climb stairs/ladder, Communicable Diseases and/or Pathogens, Driving motor vehicles (work required) *additional testing may be required, Lifting / Carrying: 0-10 lbs, Lifting / Carrying: 11-20 lbs, Lifting / Carrying: 21-40 lbs, Pushing / Pulling: 0-25 lbs, Pushing / Pulling: 26-40 lbs, Reaching above shoulder, Squat/kneel, Walking
