Overview
The Clinical Triage RN directs patients by a patient centered approach to the appropriate level of care based on the severity of their clinical need. The severity of the patient’s needs are assessed using protocols, algorithms and clinical judgement that can provide patient disposition to urgent care, emergency care, clinic provider appointments, and /or other services. The Clinical Triage RN will support communication to the patient, family and support systems as appropriate. Exhibits optimal partnership with clinical practices, contact center teams and other community services/resources. Facilitates information through appropriate communication and documentation. The Clinical Triage RN role contributes to guiding patients to the most appropriate site of care based upon their stated care needs, and the interruption of frequent acute hospital or emergency department use patterns.

Prior experience in ambulatory case management or care coordination and chronic diseases. Prior experience with EPIC Ambulatory EMR.

Health Requirements
This role is a healthcare worker position. Healthcare workers (HCW) are defined as university employees working a healthcare setting who have the potential for direct or indirect exposure to patients, human research subjects or infectious materials including body substance, contaminated medical supplies, devices and equipment, surfaces, or air. HCW have specific health requirements that must be met prior to starting work, including MMR vaccine or immunity, varicella (chickenpox) vaccine or immunity, TB screening, COVID vaccine according to University policy, hepatitis B vaccine or immunity, and annual flu vaccination.
